This registration allows him to acknowledge or file intent to acknowledge paternity, but does not in itself legally establish paternity or child support. This CPR is in compliance with 10 O.S. 7506-1.1. Instructions for Preparation of Form Only the original form is prepared. A putative father completes Section A, B and/or C as appropriate to register on the CPR or update a previous registration. A district court clerk must submit a form at the time of adjudication of a man as a legal father. The information requested on the form is self-explanatory. The father or court clerk signs and dates the form. Routing of Form The form is submitted to Oklahoma Department of Human Services at the address listed on the bottom of the form. 2 OKLA.
