Case Name: _____________________________________ Cause No:________________________ Appendix A: School Transfer Petitioner and Respondent are under the age of 18 and attend the same public or private elementary, middle or high school. The Court has entered a Sexual Assault Protection Order. The Court has considered the following factors regarding Petitioner's request that Respondent transfer to a different school: (A) The severity of the act; (B) continuing physical danger or emotional distress to the Petitioner; (C) expenses involved; and (D) the educational disruption that will be caused by a transfer of the Respondent to another school. After consideration of the above, the Court orders as follows: 1. Respondent is restrained from attending ___________________________________ School. 2. Respondent shall transfer to a different school and Respondent's parents or legal guardians are responsible for transportation and other costs associated with the change of school. 3. The Court Administrator shall send a copy of this order to the school listed in paragraph 1 above and to the new school of respondent, if known. The new school is _________________________ at ____________________________________________________________________________.
