WAIVER OF TRIAL COURT'S ATTENDANCE DURING VOIR DIRE AND THE RECORDING OF THE VOIR DIRE The defendant having reviewed with his/her attorney the right to have a judge present during jury selection does hereby knowingly and voluntarily waive the judge's presence. The defendant having reviewed with his/her attorney the right to have the jury selection recorded does hereby knowingly and voluntarily waive the need to record the jury selection. (Defendant's signature) I hereby state that subject to the penalties of Section 4904 of the Crimes Code that I have reviewed the waiver of the trial judge's attendance and the recording of jury selection with my client fully and completely and the defendant voluntarily has waived those rights.
