T he Commission will use the information provided in this form to determine whether grant of this application is in the public interest. If we believe there may be a violation or potential violation of a statute, regulation, rule or order, your application may be referred to the appropriate Federal, state, or local agency responsible for investigating, prosecuting, enforcing or implementing the statute, rule, regulation or order. FCC 405 INSTRUCTIONS April 2005 PAGE 1 American LegalNet, Inc. www.USCourtForms.com<<<<<<<<<********>>>>>>>>>>>>> 2 INSTRUCTIONS FOR COMPLETING FCC FORM 405 FOR A MULTIPOINT DISTRIBUTION SERVICE The Multipoint Distribution Service (MDS) is a domestic public radio service that provides a wide range of two-way or one-way services to a variety of users in either digital or analog modulation on microwave frequencies from a fixed station transmitting (usually in an omnidirectional pattern) to multiple receiving facilities located at fixed points. FCC Form 405 is used by Multipoint Distribution Service to apply for renewal of radio station licenses. Section 307(c) of the Communications Act limits the term of a radio license and requires that applications be submitted for renewal. FCC Form 405 is required by 47 CFR Part 21. A. Complete the application and submit two (2) copies of the completed FCC Form 405 to the Federal Communications Commission at the appropriate address noted below. The current fee amount and Payment Type Code (required for feeable applications) for Part 21 can be found in the WTB Fee Filing Guide.
