REQUIREMENTS FOR PHASE III: _______ Must report to your Probation Officer ONE time per week. _______ Must submitONE urine sample per week for analysis. _______ Must meet with the JudgONCEe every three weeks, per Judge instruction. _______ Must attend group treatmenONEt time each week. _______ May attend individual therapy sessions as needed. _______ Must attend a minimum ofTWO (2) NA or AA meetings each week. _______ Must attend a minimum ofONE meeting with your sponsor each week. _______ Must perform 40 HOURS Community Service per week if not employed, OR verifiable 40 hours per week employment/full time school or combinatio n. _______ Must pay all Drug Court Program fees. _______ Continue Lifeskills training as identified at assessmen t. <<<<<<<<<********>>>>>>>>>>>>> 2 SANCTIONS: st _______ 1 Positive UA: (1) ONE day in jail. nd _______ 2 Positive UA: (1) TWO days in jail. (2) Move back 5 weeks or to the beginning of the phase you are currently in. rd _______ 3 Positive UA: (1) FIVE days in jail. (2) Movement in phases as described above. _______ STALLS or missed UA: The sanction that would be applied for the next dirty UA. _______ EACH SUBSEQUENT POSITIVE UA: (1) FIVE days in jail. (2) Movement in phases as described above. _______ DRUG COURT FEES: Drug Court fees are $20.00 per month at $5.00 per week if you are unemployed and $40.00 per month at $10.00 per week if you are employed. _______ MISSED AA/NA, CHECK-IN: If you miss ONE AA/NA meeting or check-in in a week you are allowed to make it up by doing four (4) hours of Community Service. The Community Service must be done by close of business Tuesday of the following week and your Probation Officer must be informed by close of business Tuesday of the following week in order to receive credit and it must dbeone through the County Community Service Program. The Court retains discretion to modify these sanctioton gs ain compliance of the participants in Drug Court program, as is recommended by the Probation Officer.
