You must not be convicted of NOR RECEIVE any violations for a period of 90 days. 3. To complete disposition of the above docketed charge, YOU MUST APPEAR AFTER 90 DAYS (or send by Mail, Post Marked after 90 days) and sign a sworn affidavit that you have not been convicted of NOR RECEIVED a moving violation for the deferral period. 4. Attach a copy of (1) valid current Texas Driver's License, (2) proof of Liability Insurance. WARNING: If you fail to appear at the final appearance date in person or by mail and comply with the above requirements, you will be convicted of the offense. AFFIDAVIT: 1. I have a valid TX Driver's License or permit, and it is NOT a commercial license (copy attached), OR am a member, or the spouse or dependent child of a member, of the United States Military Forces serving on active duty (copy of Military ID attached) 2.
