That substantial consummation of the plan as contemplated in 11 U.S.C. 1127(b) and 1101(2) has occurred. 3. That the following factors, if applicable, have occurred: deposits required by the plan have been distributed; property proposed by the plan to be transferred has been transferred; the debtor or the successor of the debtor under the plan has assumed the business or the management of the property dealt with by the plan; payments under the plan have commenced; and all motions, contested matters, and adversary proceedings have been finally resolved. 4. That a final decree as contemplated in Fed. R. Bankr. P. 3002 be entered.
