Please type this form In black. 2. Enclose filing fee of $ 4.00 if form is typed, or $8.00 if not typed. For attachments, not counting any attached Form SL-2, add $1.00 per printed page. If you have a pre-paid account with the Secretary of State, enter your customer number in the block in the upper left hand portion of the form in lieu of enclosing fees. 3. File only the original. Make copies for your file. The original will be returned as your acknowledgment. 4. Check the appropriate box to indicate whether this lien is for Seed or for Farm Labor. 5. Enter only one producer name or business name per block exactly as it is to be indexed. Include any applicable titles. The term "title" as used in the producers name includes "Sr.", "Jr.", "III", "Dr.", etc. The producer is the person against whom the lien is claimed. 6. Enter the name and address of each claimant. Include ZIP Code. 7. If someone other than Claimant #1 is to receive the acknowledgment of filing, complete the "Mailing Address for acknowledgment" space. 8. Enter the Dollar amount of the claim. 9. Enter either the three-digit code for the crop from the table below, or the name of the crop, or both, in the appropriate column. Enter either the county code from the table below or the name of the county where the crop is growing or to be grown. If the crop year is not the current calendar year, enter the crop year; if the crop year is the current year, leave it blank. 10. There must be a signature for every claimant listed. A claimant may sign personally or by an agent, attorney, family member, partner, officer, etc. Enter the name of the claimant below the signature. On the third line, indicate "Self" if the signer is the claimant; if the signer is the claimants agent, attorney, parent, partner, corporate officer, etc., indicate that capacity. 11. If there are not enough lines for producers, claimants, crops or signatures on the form SL-1, enter the excess on an attached Form SL-2 supplement.
