SECRETARY OF STATE STATE OF MONTANA Bob Brown Montana State Capitol Secretary of State PO Box 202801 Helena, MT 59620-2801 Uniform Commercial Code Farm Bill FARM PR ODUCTS You may register for any farm product(s) for which you wish to receive portions of the master list by entering the farm product name in the table under the heading titled Farm Product. If you wish to receive listings for all farm products, which includes crops, livestock and un-manufactured products please enter the word "All" in the table. Note: If you order the Output Format of CD-ROM you will automatically receive all farm products. COUNTIES You may register for a specific county by entering the county name in the table under the heading titled "Counties". The Farm Bill Master List will only reflect those farm products produced and/or located in the county you indicated on your registration. Note: You may only enter specific counties if requesting the Output Format of Standard Paper. If you order the Output Format of Microfiche you will automatically receive all counties for the farm products you selected. If you order the Output Format of CD-ROM you will automatically receive all counties for all farm products. CROP YEA R You may register for a specific year by entering the year in the table under the heading titled "Year". When entering the specific year please list all four digits. Example-1994, 1995, etc. The Farm Bill M aster List will only reflect those farm products that fall within the year(s) you indicated on your registration. Note: You may only enter specific years if requesting the Output Format of Standard Paper. If you order the Output Format of Microfiche you will automatically receive all years for the farm products you selected. If you order the Output Format of CD-ROM you will automatically receive all years for all farm products. OUTPUT MEDIA You may choose between three different output medias. 1) Standard Paper is on computer paper listing active agricultural financial statements within the farm products in alphabetical or numerical order. 2) Microfiche lists active agricultural financial statements within the farm products in alphabetical or numerical order. 3) CD-ROM lists active agricultural financial statements within the farm products in alphabetical order only. Note: You may search by FIN/SSN even though the list is in alphabetical order. OUTPUT FORMA T You may choose between two different output formats. <<<<<<<<<********>>>>>>>>>>>>> 21) Alpha lists agricultural financial statements byspecific farm the pr oduct(s) in alphabetical order by debtors name. 2) Numeri lists agricultural financial statements by cal the specific farm product(s) in numerical order by the debtors Social Security Number or Tax ID Number. Note: Can not request CD by this method. MONTHS TO RECEIVE THE REPORT You can choose to receive the Farm Bill Master List only on selected months, or all months of the year. SIGNATURE For the registration to be accepted, the fomurmst be signed and dated by an authorized representative of the organization. MASTER LIST The Secretary of States office runs the Farm Bill Master List on the 15th of every Month. The list is then compiled and mailed to the Registered Buy no later thers en the 20th of every Month. If you fail to receive your Monthlylist, please call (406) 444-1212. FEES State statute requires all fees to be paid in advnce, ya ou will need to send pre-payment to start receiving your Monthly Farm Bill list. Unlike other States which have implemented a Central Filing System, there is no charge to register, amend or cancel your Buyer Registration Form. The Secretary of States office charges only for the Farm Bill Master List. Paper output will require a pre-payment of twentyfive dollars ($25.00) and microfiche output will require a pre-payment of ten dollars ($10.00). CD-ROM output will require a pre-payment of twenty dollars ($20.00). To help reduce the cost of the Farm Bill MastList, er each farm product is broken into a Product Category. The Product Categories consist of the following: Barley, Cattle and Calves, Eggs, Hay, Hogs, Horses and Mules, Milk and Dairy Products, Potatoes, Sugar Beets, Sheep and Lambs, Wheat, Wool, Other (Crops), Other (Livestock) and Other (Un-manufactured Goods). The fee for receiving the Farm ll Master List is $5.00Bi per product category, plus the volume of information you receive. The following is a list of the charges. You will pay a fee of five dollars ($5.00) per Month per product category you have selected. In addition, if you selected the Output FormatS of tandard Paper an additional charge of ten cents ($.10) a page for every page over fifty will be charged. If you selected the Output Format of Microfiche an additional charge of twenty cents($.20 ) per microfiche over twenty-five will be charged. CD-ROM is a flat $20.00 per month. AMENDING YOUR REGISTRATION You may amend your Buyer Registration Form for no charge by sending a written request of the change to the Montana Secretary of States OfficNote. e: Our office submits the Farm Bill job on the 15th of every Month which requires your request for change to be received prior to the 15th or the amended information will not appear on the Farm Bill Master List until the following Month. <<<<<<<<<********>>>>>>>>>>>>> 3CANCELING YOUR REGISTRATION You may cancel your Buyer Registration Form for no fee by sending a written request of the cancellation to the Secretary of States OfficNote. e: Our office submits the Farm Bill job on the 15th of every Month which requires your request for cancellation to be received prior to the 15th or your request for the Farm Bill Master List will not be canceled until the following Month. The Secretary of States office requires a 30 da notice ys when canceling your registration.
