(check as appropriate) 1. To waive the provision made for me in the Will and to take the share of the decedent's estate specified in 14 V.S.A.§ 319. 2. To set out the homestead pursuant to 27 V.S.A.§ 105. 3. To request that household goods and furnishings be awarded to me. A memorandum describing the household goods and furnishings that I am requesting is attached. 4. To request that support be ordered for myself and my minor children, or either. A memorandum describing the support that I am requesting is attached. This is the form by which you give notice to the Court and other interested persons of your election of certain rights that you have in the estate of your deceased spouse.
