IN THE CIRCUIT COURT OF THE SIXTEENTH JUDICIAL CIRCUIT KANE COUNTY , ILLINOIS Case No. Plaintiff/Petitioner Defendant/Respondent File Stamp PETITION FOR ORDER TO SHOW CAUSE - FAILURE TO PAY CHILD SUPPORT OR MAINTENANCE The Plaintiff Defendant now comes and petitions this court for the issuance of an orde r requiring the Plaintiff Defendant to show cause, if any, why he/she should not be held in indire ct civil contempt of this court, and in support thereof, states as follows: 1. That on an order was entered by this court, requiring the Plaintiff Defendant to pay the sum of $ per week bi-weekly month twice a month other as and for child support/maintenance; 2. That the Plaintiff Defendant has failed to comply with the terms of that order and that the re now exists an arrearage in the amount of $ ; 3. That demand has been made upon the Plaintiff Defendant for the payment of that amount, but the Plaintiff Defendant has failed to pay in accordance with the court order; WHEREFORE , the Plaintiff Defendant prays that this Court enter an Order to Show Cause requiring t he Plaintiff Defendant to appear and show cause why he/she should not be held in indi rect civil contempt of court for violating the order of this court.
