File No. STATE OF NORTH CAROLINA County In The General Court Of Justice District Court Division IN THE MATTER OF: Name And Address Of Juvenile SUPPLEMENTAL ORDER CONDITIONS OF PROBATION (DELINQUENT) Juveniles Date Of Birth Age Date Of Hearing G.S. 7B-2510 Pursuant to a disposition order entered or modified on this date, into which this Supplemental Order is incorporated by reference, the Court orders the juvenile named above, as conditions of probation, to: 1. Remain on good behavior and not violate any laws. 2. Not violate any reasonable and lawful rules of the juveniles parent, guardian , or custodian. 3. Attend school regularly. 4. Maintain passing grades in at least courses (NOTE: state number of courses up to 4) during each grading period and meet with the court counselor and a school representative to make a plan for maintaining passing grades. 5. Not associate with (specify person): 6. Not be in the following place(s): 7. Not use or possess any controlled substances included in any schedule of Article 5 of Chapter 90 of the General Statutes, the Controlled Substances Act. 8. Not use or possess any alcoholic beverage regulated under Chapter 18B of the General Statutes. 9. Submit to random drug testing. 10. Abide by the following curfew: 11. Submit to a warrantless search at reasonable times. 12. Possess no firearm, explosive device, or other deadly weapon. 13. Report to a court counselor as often as required by the court counselor. 14. Participate in a life skills or an educational skills program administered by the Office of Juvenile Justice. 4. Cooperate with electronic monitoring.
