Other: (specify) AOC-J-251, New 7/99 (Replaces J-501) 1999 Administrative Office of the Courts (Over) <<<<<<<<<********>>>>>>>>>>>>> 2 CONCLUSIONS OF LAW The Court concludes as a matter of law that the following disposition is legally authorized and is an appropriate plan to meet the needs of the juvenile and to achieve the objectives of the State in exercising jurisdiction. ORDER It is therefore ORDERED that: 1. Continuance For Family To Implement Approved Plan [G.S. 7B-2501(d). The case be continued for , to allow the juveniles family an opportunity to meet the juveniles needs through the following plan, which the Court approves: 2. Examination By Expert [G.S. 7B-2502(a)]. The juvenile be examined by (specify name or type of expert) , to assist the Court in determining the juveniles needs, and that a hearing be held on (specify date) , to determine whether the juvenile is in need of evaluation or treatment and, if so, who should pay the cost; further, that the Clerk send notice of that hearing to the county manager or other person designated by the chair of the board of county commissioners of the county named above. NOTE TO CLERK: Mail notice as provided above. 3. Interdisciplinary Evaluation [G.S. 7B-2502(a)]. The area mental health, developmental disabilities, and substance abuse services director arrange an interdisciplinary evaluation of the juvenile, mobilize resources and take other appropriate action to meet the juveniles needs, and report to the Court. Further, that the Clerk mail a copy of this Order to the area mental health director. NOTE TO CLERK: Mail copy of order as provided above. 4. In-Home Supervision With Conditions [G.S. 7B-2503(1)a.]. The juvenile remain at home and be supervised by , subject to the following conditions applicable to the parent, guardian, custodian, and/or the juvenile, as specified: 5. Change Of Custody [G.S. 7B-2503(1)b.]. The juvenile is hereby placed in the custody of: a. The juveniles mother, . . b. The juveniles father, c. The juvenile is placed under the protective supervision of a court counselor for (specify time period, up to three (3) months) and as a condition of protective supervision is ordered 1. Remain on good behavior and not violate any laws. 2. Attend school regularly. 3. Maintain passing grades in at least courses (NOTE: State number of courses up to 4) during each grading period and meet with the court counselor and a school representative to make a plan for maintaining those passing grades. 4. Not associate with (specify person(s)): 5. Not be in the following place(s): 6. Abide by the following curfew: 7. Report to a court counselor as often as required by the court counselor. 8.
