File No. STATE OF NORTH CAROLINA In The General Court Of Justice County District Court DivisionName And Address Of Plaintiff ORDER APPROVING VERSUS Name And Address Of Defendant PARTIAL PARENTING AGREEMENT G.S. 50-13.1Name And Address Of Plaintiffs Attorney Name And Address Of Defendants Attorney The Court finds the following facts: This matter involves issues relating to child custody or visitation. The parties were ordered to participate in the courts mediation program for custody and visitation disputes. The parties were successful in reaching a mediated partial parenting agreement. The parties have been provided an opportunity to review their partial parenting agreement with their attorneys, if any, and anyone else of their choice. Each party has now acknowledged that the court should enter an order making their partial parenting agreement an order of the court. The parties partial parenting agreement has been reviewed by the court. No good reason has been shown as to why the partial parenting agreement should not be incorporated into an order of the court. The parties have not been able to agree upon the following issues: (attach additional sheets if necessary) The Court further finds that for the six (6) months next preceding the filing of this action: The plaintiff was a resident of (name state) . The defendant was a resident of (name state) . The child(ren) was/were residents(s) of (name state) . Other findings: Based on the foregoing findings the Court concludes that it has jurisdiction of the parties and subject matter, and that it is authorized to make a child custody determination under the provisions of G.S. 50-13.5(c) and G.S. 50A-3(1). The Court further concludes that it would be in the best interests of the child(ren) that the partial parenting agreement of the parties be incorporated into an order of the court. It is ORDERED that the parties partial parenting agreement, a copy of which is attached, is incorporated and becomes the order of this Court. The parties may by mutual agreement re-enter the courts mediation program without a further court order, provided that any modification agreement be submitted to the court for its approval. A filed copy of this Order shall be mailed to the parties and their counsel. ALL PARTIES ARE NOTIFIED that it is a FELONY for any person, with the intent to violate this Court Order, to take or transport, or cause to be taken or transported, (any of) the minor child(ren) included in this Order, outside this state, or to keep the child(ren) outside this state and such action shall subject any such person to criminal prosecution under the provisions of G.S. 14-320.1.
