(a) 99 The plaintiff is now receiving or has received public assistance toward the support of the above-named minor child(ren) from the State of Alabama, Department of Human Resources, and has assigned, by operation of law, support rights in regard to the child(ren) to the State of Alabama, Department of Human Resources, the payments being an obligating due to the State of Alabama OR (b) 99 Is now receiving services from the State of Alabama for the enforcement of support pursuant to Title IV-D of the Social Security Act, as amended. 3. The plaintiff is now receiving or has received medical benefits from the State of Alabama, Department of Human Resources, the medical benefits being an obligation due to child(ren). 4. The defendant is the person charged with the legal duty of supporting the child(ren) and has not supported the child(ren). No support payments have been previously ordered pursuant to a decree of divorce. WHEREFORE, the premises considered, the plaintiff moves this Honorable Court as follows:1. To enter an order setting a hearing on the Plaintiffs complaint to establish the paternity of and the amount of support due on behalf of the child(ren) named above; 2. To enter an order adjudicating the paternity of the child(ren) named above and directing the Defendant to pay a reasonable sum toward the support and maintenance of the minor child(ren), and enter a withholding order.3. To require the payments to be made payable to the Clerk of this Court or to the appropriate agency designated by the Court to be transmitted to the State of Alabama, Department of Human Resources, Montgomery, Alabama, where applicable; 4. To require the Defendant, wherever employed, to include the child(ren) named above on any medical insurance policy of medical insurance coverage at his or her place of employment or to include the child(ren) named above on any medical insurance policy or medical insurance coverage which the Defendant may purchase and to provide the evidence of such coverage to the State of Alabama, Department of Human Resources. 5. To require the Defendant to reimburse the State of Alabama $ for public assistance previously paid; 6. To require the Defendant to reimburse the State of Alabama $ for medical benefits previously paid; 7.
