Defendant Case No. Address Application for Sealing of Conviction Now comes the defendant in the above -entitled case and represents to the Court that he/she has been convicted of a misdemeanor case in Summit County, City of Ohio, to - wit: on the day of , 20 , he/she was convicted of or plead guilty to: and that his/her probation was terminated or became final on, or that hi s/her final discharge from the Court was on , which is more than one yea r prior to the date of this filing. Defendant further states that this is the only misdemeanor offense for w hich he/she has ever been convicted in any jurisdiction, and that there is no w no criminal charge pending against him/her in this or any other juri sdiction. Defendant further requests that a date of hearing be set upon such motio n and that an investigation be conducted pursuant to the statute to determine that defendant has attained rehabilitation to the satisfaction of the Court.
