Parent respectfully represents: 12 1. The value of the assets of the conservatorship is $ , 13 which is less than $10,000. A copy of the most recent account statement is attached. 14 2. Parent wishes to transfer the entire assets of Account # 15 at [name of financial institution holding16 conservatorship fundsname of financial institution holding 17 provide that no funds shall be withdrawn without prior court approval.
