COURT PROGRAM Malheur County Courthouse 251 B Street West - Vale, OR 97918 541/473-5178 / / Case # & Participant Name Date STRICT COMPLIAN CE PROGRAM YOU HAVE FAILED TO COMPLY WITH THE S.A.F.E. COURT PROGRAM AND ARE ON THE VERGE OF BEING TERM INATED FROM THE P ROGRAM . YOU ARE THEREFORE, PLACED ON THE FOLLOWING STRICT COMPLIANCE PROGRAM: 1. No positive urinalysis (UAs) for illegal drugs, no possession of restricted or illegal drugs or positive intoxilyzer results for alcohol. 2. No missed UA or intoxilyzer dates. 3. No missed group sessions or community support meetings, such as AA or NA or similar meetings. 4. No late appearances for group or other meetings, and court appearances. 5. No missed court appearances. 6. Weekly reports from Lifeways. 7. Appearances before the court every two (2) weeks. Must appear for Sanction weeks. 8. No drinking of any alcoholic beverages, including so-called non-alcoholic beer or wine. 9. No use of narcotic or alcohol based medications or cold remedies. Ask your doctor or your treatment provider for an alternative. This procedure will remain in place for at least 60 days, or until graduation or a request ismade by the counselor and/or probation officer to remove the Strict Compliance provision and allow the regular program to be reinstated. NOTE: Any individual who fails this Strict Compliance program will be terminated from S.A.F.E. Court. This is a last-ditch effort to try to get compliance and a successful completion of this program by the process of a graduation.
