ORDER APPOINTING GUARDIAN AD LITEM On this _____ day of , 20____, the Court considers the Application To Appoint A Guardian Ad Litem, for the above-named Ward and the evidence presented and finds that a Guardian is unavailable and that a Guardian Ad Litem should be appointed to protect the Ward. WHEREFORE, IT IS ORDERED that is hereby appointed the Guardian Ad Litem for until the Guardian is again available, but for a period not to exceed days.
