Within 10 calendar days of filing an appeal to the Court of Special Appeals from a Circuit Court civil case or in an Orphans' Court case, the person who filed the appeal must complete, sign, and mail or deliver this form to the Clerk's Office of the Court of Special Appeals (address above) and send copies of the signed, completed form to all other attorneys and unrepresented parties in the case.
