Instructions for QUIT -CLAIM DEED Before recording this Quit-Claim Deed, it must be filled out in its entirety, signed by the appropriate parties, it must bear the signature of two witnesses present at the signing of this deed before a notary, along with the Notarys signature and official stamp. "Prepared by"-This information must include the name and address of the person who prepared this document. "Return to"-The name and address of the person who we are to mail the recorded document back to. (Please note, you can ask for a 5 Minute return and we will give you your original recorded document back to you at the time of recording for a $1.00 per page fee.) "Grantor"-this is the party(ies) who are transferring ownership of the property. They are the only people required to sign this document. "Grantee"-this is the party(ies) who is receiving ownership of the property. Their signature is not required on this form. The signatures of the Grantors and the witnesses must also be wrote out in printed form underneath the signed signature. You will also need to fill out a Transfer of Interest form, (a/k/a DR-219). This form is available in the Recording Department. There are instructions on the back of the DR-219 form. This form is also available on the web at www.state.fl.us/dor/forms. From that page, click on "Download current forms". Where it is asking you to "Choose a Tax Category", use the down arrow at the right side of the field and click on "Documentary Stamp Tax", then click on "Get Forms". At the next screen, click on DR-219". You should be able to print it out from there. You will need to know the Parcel Identification number of the parcel of land of which you are transferring ownership. This is a 17-digit number and can be found on your tax bill. If you do not have a copy of your tax bill you can find the number on the Tax Rolls located in the vault in the Recording Department.
