PROBATE COURT OF SUMMIT COUNTY, OHIO ADOPTION OF: CASE NO. ENTRY ON PLACEMENT PUTATIVE FATHER REGISTRY (R.C. 3107.07(B)) This cause came on to be heard on the application of , the mother of , and for approval of the proposed placement of said child with a necessary party to this action at this date. WHEREUPON, there appeared before the Court the mother of said child, who was examined by the Court; and there was submitted to the Court the report of , the assessor appointed to perform a homestudy and who has recommended the proposed placement.
