PROBATE COURT OF SUMMIT COUNTY, OHIO ADOPTION OF: CASE NO. DETERMINATION OF SUITABILITY AND JUDGMENT ON PLACEMENT This cause came on to be heard on the application and , of Husband and Wife, Applicants herein, for the approval of the proposed placement of who presently reside(s) outside the territory of the United States, with for adoption; and, for the purpose of certification to the Department of Immigration and Naturalization of the United State to comply with the requirements of Title 8, U.S.C.A. Aliens and Nationality 1101 (b)(1)(F). WHEREUPON, and , Husband and Wife, submitted to the Court a report of , who was heretofore appointed to make an independent investigation of the proposed placement; and the Court finds, after consideration of the investigative report and evidence submitted, that the applicants and , are suitable and qualified persons who meet the high requirements as adoptive parents and that this proposed placement would be in the best interest of the child(ren). IT IS, THEREFORE, ORDERED by this Court that said proposed placement be and is hereby approved of record; that based upon the determination that the applicants are suitable, a certified copy of this order be delivered to the United States Department of Immigration and Naturalization as required in matters of this nature.
