CSD 4025 [01/06/00] UNITED STATES BANKRUPTCY COURT SOUTHERN DISTRICT OF CALIFORNIA 325 West F Street, San Diego, California 92101-6991 DOCUMENTS REQUIRED FOR PROCESSING AN APPLICATION FOR UNCLAIMED FUNDS In order to timely process your application for funds you may be entitled to, the Court requires the original and two (2) copies of the documents listed below. In addition, a self-addressed stamped envelope must be submitted. IF CLAIMANT IS ASSISTED BY A FUNDS LOCATOR, you must submit all of the following: Completed APPLICATION FOR RETURN OF TENDERED FUNDS, (CSD 4014) Completed PROOF OF RIGHT TO PAYMENT OF DIVIDEND FROM UNCLAIMED FUNDS, (CSD 4026) Completed Limited Power of Attorney form (attach to APPLICATION) IF CLAIMANT REPRESENTS A CORPORATION, you must submit all of the following: Completed APPLICATION FOR RETURN OF TENDERED FUNDS, (CSD 4014) Completed PROOF OF RIGHT TO PAYMENT OF DIVIDEND FROM UNCLAIMED FUNDS, (CSD 4026) Copy of Certificate/Article of Incorporation (attach to PROOF OF RIGHT form) OR Certificate/Articles of Merger or Successor Corporation (attach to PROOF OF RIGHT form) IF CLAIMANT IS AN INDIVIDUAL, you must submit all of the following: Completed APPLICATION FOR RETURN OF TENDERED FUNDS, (CSD 4014) Completed PROOF OF RIGHT TO PAYMENT OF DIVIDEND FROM UNCLAIMED FUNDS, (CSD 4026) Photocopy of current driver's license (attach to PROOF OF RIGHT form). If you do not have a current driver's license, other identification containing a photograph and current address is required.
