THE UNITED STATES BANKRUPTCY COURT FOR THE DISTRICT OF COLUMBIA IN RE: Case No: Chapter: Debtor(s) ISSUED TO: NOTICE TO CONSUMER DEBTOR In accordance with 11 USC 342 (b), "Prior to the commencement of a case under this title by an individual whose debts are primarily consumer debts, the clerk shall give written notice to such individual that indicates each chapter of this title under which such individual may proceed." A copy of said notice shall be signed by the debtor(s) and shall be filed contemporaneously with the petition.
