UNITED STATES BANKRUPTCY COURT EASTERN DISTRICT OF CALIFORNIA DOCUMENTS REQUIRED FOR PROCESSING AN APPLICATION FOR PAYMENT OF UNCLAIMED FUNDS Applications for payment of unclaimed funds will not be processed unless accompanied by the following documents: IF THE OWNER OF THE FUNDS IS AN INDIVIDUAL, submit all of the following: Completed APPLICATION FOR PAYMENT OF UNCLAIMED FUNDS (EDC 3-950); Completed INDIVIDUAL IDENTIFICATION FORM FOR UNCLAIMED FUNDS (EDC 3-951); Photocopy of current driver's license (attach to IDENTIFICATION FORM). If you do not have a current driver's license, other identification containing a photograph and current address is required; and Proposed ORDER FOR PAYMENT OF UNCLAIMED FUNDS (EDC 6-950). IF THE OWNER OF THE FUNDS IS A BUSINESS OR CORPORATION, submit all of the following: Completed APPLICATION FOR PAYMENT OF UNCLAIMED FUNDS (EDC 3-950); Completed CORPORATION/BUSINESS IDENTIFICATION FORM FOR UNCLAIMED FUNDS (EDC 3-952); Certified copy of Certificate/Articles of Incorporation, certified copy of Certificate/Articles of Merger or Successor Corporation, meeting minutes or other documents (i.e. Affidavit of the Chief Executive Officer or other corporate officer) identifying the representative and establishing that they are a duly authorized representative of the corporation (attach to IDENTIFICATION FORM); Photocopy of representative's identification credentials (attach to IDENTIFICATION FORM); and Proposed ORDER FOR PAYMENT OF UNCLAIMED FUNDS (EDC 6-950). IF THE OWNER OF THE FUNDS IS REPRESENTED BY A FUNDS LOCATOR, submit all of the following: Completed APPLICATION FOR PAYMENT OF UNCLAIMED FUNDS (EDC 3-950); Completed INDIVIDUAL IDENTIFICATION FORM FOR UNCLAIMED DIVIDENDS (EDC 3-951) or CORPORATION/BUSINESS IDENTIFICATION FORM FOR UNCLAIMED FUNDS (EDC 3-952) and supporting documentation; Original Power of Attorney that includes the claimant's notarized signature (attach to APPLICATION); Photocopy of the claimant's current driver's license or representative's identification credentials (attach to IDENTIFICATION FORM); and Proposed ORDER FOR PAYMENT OF UNCLAIMED FUNDS (EDC 6-950).
