I then left a message with the person answering the phone that I would be applying in writing to the San Joaquin Superior Court at the address noted above for an order appointing temporary guardian. I asked that the message be delivered as soon as possible. (and/or) I have not informed certain persons listed in item three (3) (indicate person by letter) and request that I not be required to for the following reasons: 3. A. C. B. D. 4.
