FORM SUMMARY Name of Form: Order Concerning Petition to Expunge Court Record of Adjudication JD-1781 938.355(4m) JV 11 & 12 To provide a formal order on a petition to expunge the court record of a juvenile's adjudication of delinquency. The juvenile court clerk for signing by the court official. Original is filed with the court. Copies to the petitioner and district attorney. Generally none. Modification, last update 7/96. Added a file/date stamp to upper right corner. Added a statement on the bottom indicating that the form shall not be modified. A juvenile adjudicated delinquent, after reaching age 17, may petition the court to expunge the court record of the adjudication. The court may expunge the court's record of adjudication if it finds that: · the provisions of the dispositional order have been satisfactorily completed; and, · the juvenile will benefit; and, · society will not be harmed by the expungement. This form may be used either with or without a hearing, depending on the court prerogatives based on the district attorney's recommendation. Presumably, however, if the petitioner files the petition even with a district attorney disapproval, the petitioner is probably entitled to a hearing on the matter. About this form: This form is the product of the Wisconsin Records Management Committee, a committee of the Director of State Court's Office and a mandate of the Wisconsin Judicial Conference.
