The Depositor's Authorization to Transfer Funds for Support, (Not an RMC form) provided by the Department of Health and Social Services may be attached to the copy sent to the payer. Modification, last update 10/96. Added a file/date stamp to upper right corner. Added a statement on the bottom indicating that the form shall not be modified. Previous comments: If an income withholding order under s. 767.265, Wisconsin Statutes, would be inapplicable, ineffective, or insufficient to pay child support, the court is authorized to order a payer to identify or establish a deposit account from which funds can be transferred to pay the support. The order directs the payer to identify and, if necessary, create such an account, and to complete the Depositor's Authorization to Transfer Funds for Support [This Authorization is not an RMC form]. The Authorization is sent to the financial institution for their records and to start the transfer process. RMC recommends that the court not file a copy of the Authorization in the court records because of the sensitive and confidential nature of the financial account information contained on the form. Effective 1/1/96, sections 767.25(4m)(c) and 767.51(3m)(c) Wisconsin Statutes become effective. Beginning on that date, similar orders will be allowed to payment of a child's health care expenses, including payment of health insurance premiums. This form can continue to be used for such without modification.
