SUPP. 1985, ' 84.1) State of Oklahoma ) ) ) ss. , of lawful age, first being duly sworn, deposes and says: That I am an employee of and was subpoenaed as a witness to testify in the above-styled criminal case on a matter pertaining to my employment. Further, that pursuant to 28 O.S. Supp. 1985, ' 84.1 and 74 O.S. Supp. 1991, ' 500.1 et seq., I am entitled to reimbursement for the following: (Circle the number of the item(s) being claimed) 1. 2.
