OCCAOnline Rules of the Court of Criminal Appeals SECTION XIII. FORMS Rule 13.0 Mandatory Forms to be Utilized in Criminal Cases in the State of Oklahoma The following forms shall be utilized by trial courts and parties in the prosecution and appeal of criminal cases in the State of Oklahoma. The forms may be computerized with additional lines and spaces added as needed. In addition, appendices/ exhibits/attachments may be utilized to supplement the forms (for example - rules and conditions of probation, payment schedule for fines and/or costs, etc.).
