The Petitioner is an interested person and has filed a complete and verified Petition. 2. Any required notices have been given or dispensed with. 3. The appointment of a Guardian ad Litem is necessary to represent the interests of: Name (if applicable) a minor whose date of birth is: (date of birth) . an incapacitated or protected person (within the meaning of G.L. c. 190B). an unborn person. a person whose identity or location is unknown. a designated class of persons who are not ascertained or not in being. 4. OR The Guardian ad Litem shall be: for the following reasons: Name The Guardian ad Litem shall be selected from the Court-approved list. 5. The trust estate is adequate to cover the fees and costs of the Guardian ad Litem. THE COURT DECREES AND ORDERS: 1.
