The F.P.U. Center will be contacting you by phone within three to five days of submission of this form, to schedule mediation services. MEDIATION DATE/TIME: REFERRAL DATE: No fees will be charged to any litigant filing pleadings under a court fee waiver for up to two (2) hours of mediation including preparation of a mediated agreement should one be reached. Litigants without a fee waiver will pay a fee based on a variable fee schedule. Litigants with a fee waiver will pay for requested mediation time beyond the first (2) two hours at a rate based on a variable fee schedule.
