If a foreign company authorized to transact business in the State of Alaska is party to an organic change permitted by the laws of the home state, the surviving company shall, within 30 days file with the Commissioner a certified copy of the Articles of Conversion from the home state, along with an application for Certificate of Foreign Registration. NOTICE: The Notification of Conversion must be filed in conjunction with a Certificate of Registration of Foreign Limited Liability Company and the applicable $350.00 fee see form 08-497. ITEM 1: Provide the name of the entity immediately before filing of the certificate of conversion to a limited liability company i.e. prior to conversion. ITEM 2: Provide the name of limited liability company as listed on the attached Certificate of Registration.
