If there is a written lease, attach a copy of the lease to this complaint. On or about (date) the plaintiff (landlord), and the defendant (tenant) agreed orally or in a written lease (copy attached) that the defendant would rent the following premises (rented property): Location of apartment (Number, street, town and apartment or floor number) for the term of one (week/month/year) (term of lease). weekly (Amount of money defendant agreed to pay) 2.
