FLNB Local Form A-8 (02/13) UNITED STATES BANKRUPCTY COURT NORTHERN DISTRICT OF FLORIDA REQUEST FOR COPIES All requests for copies must be made in writing; telephonic requests will not be accepted. This form must be filled out in its entirety and submitted to the Clerk's Office with the appropriate payment (see fees below). Copy work will be completed within two (2) days of receipt of the request. A "One Time Credit Card Authorization" form is located on our website at www.flnb.uscourts.gov . You may fax this form and the one time authorization to charge your credit card to 850-521-5004 or mail this form and payment to the address provided below. Checks must be made payable to "Clerk, U.S. Bankruptcy Court." Be advised that we cannot accept checks or credit card authorization forms from debtors in pending bankruptcy cases. PAYMENT MUST BE MADE IN ADVANCE. Note that copies are also available from the public terminals located in the lobby of each U.S. Bankruptcy or U.S. District Courthouse in the Northern District of Florida at a charge of $.10 per page. There is no search fee required for obtaining copies from the public terminals.
