District of Columbia Government Corporations Division Revocation of Dissolution of Domestic For-Profit Corporation Form DBU-9, Version 1, January 2012. This form will allow for a corporation to revoke its dissolution within 120 days of its effective date. ( Revocation of dissolution shall be authorized in the same manner as the dissolution was authorized unless that authorization permitted revocation by action of the board of directors alone, in which event the board of directors may revoke the dissolution without shareholder action. Revocation of dissolution shall be effective upon the effective date of the articles of revocation of dissolution. ENTITY TYPE FILING FEE Domestic Filing Entity Refer to Corporate Fee Schedule posted online; Under the provisions of the Title 29 of D.C. Code (Business Organizations Act), the domestic filing entity listed below hereby applies for a Certificate of Revocation of A Dissolution and for that purpose submits the statement below. 1. The name of the corporation. 2. The effective date of the dissolution that was revoked. 3. The date that the revocation of dissolution was authorized. 4. Revocation has been decided according to the one of the following. (select 4A or 4B or 4C) 4A. The corporation's board of directors, or incorporators, revoked the dissolution. 4B. Revocation was permitted by action by the board of directors alone pursuant to shareholder's authorization. 4C. Shareholder action was required to revoke the dissolution and the proposal to dissolve was duly approved by the shareholders in the manner required by § 29-312.03. and by the articles of incorporation.
