District of Columbia Government Corporations Division Listing of Commercial Registered Agent Form RA-1, Version 1, January 2012. Use this form to become a commercial registered agent in the District of Columbia. "Commercial registered agent" means a person (an individual or domestic or foreign filing entity) who have filed this form as required under § 29-104.05 with Corporations Division. Commercial registered agent can be an individual or a domestic or foreign filing entity. If commercial registered agent is the filing entity then said entity must be qualified to conduct business in the District of Columbia. ENTITY TYPE FILING FEE Commercial Registered Agent Refer to Corporate Fee Schedule posted online. Under the provisions of the Title 29 of D.C. Code (Business Organizations Act), the following individual or domestic or foreign filing entity qualified to do business in the District hereby applies for a Listing of Commercial Registered Agent and for that purpose submits the statement below. 1.1. Commercial Registered Agent's name. 1.2. Commercial Registered Agent's alternate name. (alternate name shall be used only if true name is not distinguishable) 3. Individual or Filing Entity is in the business of serving as a commercial registered agent in the District. 4. Principal Address. If commercial agent is the filing entity, answer 5A-B. 5A. Entity's State of Formation. 6. Commercial Registered Agent's Address in the District of Columbia. 5B. Entity Type & File Number. 7. Name and file numbers of entities represented by the commercial registered agent. Use this form to terminate the listing of commercial registered agent. A commercial registered agent termination statement shall be effective at 12:01 a.m. on the 31st day after the day on which it is delivered for filing. The commercial registered agent promptly shall furnish each entity represented by the agent notice in a record of the filing of the commercial registered agent termination statement. When a commercial registered agent termination statement takes effect, the commercial registered agent ceases to be an agent for service of process on each entity formerly represented by it.
