District of Columbia Government Corporations Division Certificate of Series Designation for Domestic Limited Liability Company Form DLC-4, Version 3, January 2012. Use this form to establish series within the limited liability company. The operating agreement may establish one or more designated series of members, managers, or interests of a limited liability company, in which the members, managers, or interest holders have separate rights, powers, or duties with respect to specified property or obligations of the limited liability company. Code (Business Organizations Act), the domestic filing entity listed below hereby applies for Certificate of Series Designation and for that purpose submits the statement below. 1.1. Name of Limited Liability Company. 2. The date of filing of its initial certificate of organization. 3. A different name for each series that contains the entire name of the limited liability company but otherwise complies with §§ 29-103.01 and 29-103.02(f). (may include the statement) 3.4. A street and mailing address of the principal office and name and mailing address of a registered agent, if either is different from that specified for the limited liability company. 4. 5. Effective Date.
