Use this form to restate articles of organization for a domestic limited cooperative association. ENTITY TYPE Domestic Limited Cooperative Association. Code (Business Organizations Act), the domestic filing entity listed below hereby applies for Restated Certificate of Organization and for that purpose submits the statement below. 1. A limited cooperative association, by the affirmative vote of a majority of the board of directors taken at a meeting for which the purpose is stated in the notice of the meeting, has adopted restated articles of organization that contain the original articles as previously amended. 6. Restated articles that contain amendments are adopted in the same manner and with the same vote as required for amendments to the articles under § 29-1004.05(a).
