This form will allow a domestic nonprofit corporation to dissolve and wind down its business. A corporation shall be dissolved upon the effective date of its articles of dissolution. The name of the corporation. 2. The date dissolution was authorized. 3. The dissolution was approved in the manner required by § 29-412.03 and by the articles of incorporation and bylaws.
