IT IS FURTHER ORDERED THAT: 1. __________________________________________ is appointed as the developmental services decisionmaker for the named child. The above-named developmental services decisionmaker shall have the authority to access the child's information and records pursuant to subdivision (u) of Welfare and Institutions Code Section 4514 and subdivision (y) of Section 5328, and to act on the child's behalf for the purposes of the individual program plan process pursuant to Sections 4646, 4646.5, and 4648 and the fair hearing process pursuant to Chapter 7 (commencing with Section 4700) of Division 4.5, and as further set forth in this court order. The above-named developmental services decisionmaker will have access to the child and may confer with the child's social worker, probation officer, and/or attorney. The social worker, probation officer, and/or attorney may release information and/or documents that s/he deems necessary to the work of the developmental services decisionmaker. A copy of this order will be the only authorization necessary for such purpose. The above-named developmental services decisionmaker will not disclose the information received to anyone other than the assigned probation officer, social worker, attorneys, Regional Center staff, or the court unless specifically authorized by the court.
