Please take a few minutes to complete this survey regarding your experience with this mediation. Confidentiality The completed surveys are available only to the Court's ADR Program staff. Your responses will be used, in combination with the responses of others, to generate information concerning the value of the Court Mediation Panel and how it can be improved. Completing and Returning the Survey Please return the completed survey, with the Mediation Attendance Sheet and Confidentiality Agreement, to the ADR Program Office, U.S. Courthouse, 312 N.
