The petitioner asserts to the following: 1. That the petitioner was charged with _____________________________________________and adjudicated delinquent of_______________________________________________________________________, a sex offense(s) in__________________________________________________case numbers:________________ (name of county and state or jurisdiction) ______________________________________________________________________________________. 2. A certified copy of the adjudication or conviction requiring registration, which includes a detailed description of the sex offense; b. Proof of the age of the victim and the age of the sex offender at the time of the commission of the sex offense; c. A list of each county, municipality, and jurisdiction where the petitioner is required to register or has ever been required to register as a sex offender; d. A copy of the petitioner's criminal record; and e.
