A Confidential Guardianship Status Report (JC Form #GC-251) has been enclosed with this notice. You are required to complete and return the Report back to Probate Court at the address indicated above. You must complete and return the Confidential Guardianship Status Report on or before . If you do not return the annual status report, you may be subject to further order of the court which may include removal as the guardian and sanctions. Willful submission of false information is grounds for removal and is a misdemeanor. (Prob. Code § 1513.2(a).) Upon review of your Confidential Guardianship Status Report, the court may determine that it is necessary to set the matter for a review hearing. If a review hearing is necessary, you will receive a separate notice indicating the date and time at which you must appear.
