IN THE COURT OF COMMON PLEAS ASHLAND COUNTY, OHIO DOMESTIC RELATIONS DIVISION _____________________________, Plaintiff/Petitioner, Case No. ___________________ vs./and _____________________________, Defendant/Petitioner. ORDER FOR GENETIC TESTING This case came before the Court on _________________________, 20_______. The _________________________________ requested that the Court order genetic paternity testing. The Court FINDS that request to be well-taken and determines that genetic paternity testing shall be completed in this case. Despite any order herein requiring one party to advance the costs of genetic testing, the Court reserves jurisdiction to order either party to reimburse the other for a portion of the costs of the testing. It is therefore ORDERED as follows: 1. Both parties and the minor child(ren) shall report to the Ashland County Child Support Enforcement Agency, 15 West Fourth Street, Ashland, Ohio on __________________________ at _________________ ___.M. for the purpose of genetic parentage testing. 2. Each party shall bring the following documents to the CSEA: a. The Plaintiff/First Petitioner shall complete a IV-D application if she has not already done so. 4. 5. Paternity testing shall thereafter proceed as directed by the CSEA. The CSEA shall have the authority to reschedule the agency appointment and testing. In the event CSEA reschedules those events, the parties and the minor child shall appear at such time and place as directed by the CSEA.
