Petitioner: Date of Birth: COURT USE ONLY Case Number: Division Courtroom ORDER OF EXPUNGEMENT OF RECORDS JUVENILE "JD" CASE ORCRIMINAL "CR" CASE This matter having come before this Court on the Petitioner's request for expungement of records, the Court finds: Notice, as required by §19-1-306(5)(b), C.R.S., has been given to the prosecuting agency and other interested persons determined by the Court. The statutory time requirements have been met. The Petitioner has not been convicted of a felony or a misdemeanor and has not been adjudicated a juvenile delinquent since the termination of the court's jurisdiction or the Petitioner's unconditional release from parole supervision. There are no felony, misdemeanor, or delinquency actions pending or being instituted against the Petitioner. The Petitioner has been rehabilitated to the satisfaction of the Court. The expungement is in the best interests of the Petitioner and the community. The Court Orders That: 1. The following record(s) can be expunged. Date of Offense Charge Agency Case Number Arresting Agency 2. With the exception of the provisions in paragraph 3 below, upon any inquiry in this matter, all persons in charge of expunged records shall reply that no record exists. 3. The records shall be available to any judge or probation department for use in any future juvenile or adult sentencing hearing regarding the Petitioner.
