FL09069 - CAREGIVER'S AUTHORIZATION AFFIDAVIT. This is a Yuba County Superior Court informational form authorized under California Family Code §§ 6550–6552. This affidavit allows a caregiver who is at least 18 years old and with whom a minor resides to authorize school enrollment and school-related medical care by completing items 1–4. If the caregiver is a qualified relative (such as a grandparent, aunt, or uncle) and completes items 1–8, the affidavit also authorizes broader medical and dental care similar to a legal guardian’s authority under Probate Code § 2353. The affidavit is not filed with the court and does not grant legal custody. It must be signed under penalty of perjury.
