PRO-023 - CONSERVATORSHIP CARE PLAN. This is a Los Angeles County Superior Court probate form used by a conservator to outline the care, living arrangements, and financial management plan for a conservatee. The form requires detailed information about the conservatee’s residence, level of care, medical condition, medications, service providers, visitation plans, and support systems. It also includes a comprehensive financial overview, including monthly income, expenses, insurance coverage, property interests, trusts, and anticipated asset sales.
