L2752 - ATTENDANCE SHEET FOR ALTERNATIVE DISPUTE RESOLUTION (ADR) SESSION. This is an Orange County Superior Court form used to document participation in mediation or neutral evaluation sessions. The form records the names, contact information, and roles of attorneys, represented parties, and other attendees. The ADR neutral must maintain the completed attendance sheet for three years after the session date. Importantly, the form is not filed with the court and may require multiple pages if additional attendees are present.
